HEALTH PROMOTION SPECIALIST

OVERVIEW

The Health Promotion Specialist will implement programs and interventions addressing health disparities, promoting health education and reducing risk behaviors among LGBTQ+ individuals across the region.  A primary role of the Health Promotion Specialist is to screen and assess clients for Ryan White Case Management and enroll or refer to other HIV services and programs throughout the region.  The Health Promotion Specialist will also facilitate evidence informed interventions to reduce the impact and transmission of HIV in the region.  This is a full-time position reporting to the Director of Community Services.  The minimum education requirement is a bachelor’s degree in a human service field.  Prior experience working with LGBTQ+ and/or HIV/AIDS communities is preferred. 

Key Performance Area: HIV Case Management

Performance Standards: the Health Promotion Specialist will:

  1. Collaborate and consult with clinical staff and Prevention Services Coordinator to identify clients who could benefit from services
  2. Works across the regions HIV/AIDS systems to outreach to PLWHA to increase the number of clients who have access to services
  3. Screen clients to determine eligibility
  4. Develop a comprehensive assessment to determine client’s strengths and needs within 30 days of screening
  5. Facilitate a service coordination plan with action steps, goals, referrals and target dates in collaboration with clients
  6. Build relationships with other service providers in the region to establish referral agreements in service of clients’ needs
  7. Meet with clients regularly and develop plans to re-engage clients as needed
  8. Create and maintain client files documenting eligibility and goal progress

Key Performance Area: Facilitating Interventions

Performance Standards: the Health Promotion Specialist will:

  1. Provide Health Education to eligible clients in support of the care continuum 
  2. Facilitate individual and group interventions with adherence to the curriculum
  3. Conduct outreach and recruitment efforts to identify program participants
  4. Motivate and effectively engage participants to identify and reduce risk behaviors
  5. Deliver interventions without judgments or discrimination
  6. Identify and utilize current best practice recommendations and incorporate in facilitation 

Key Performance Area: Health Promotion & Community Events

Performance Standards:  The Health Promotion Specialist Educator will:

  1. Represent the agency at community events providing literature and information to improve well-being
  2. Encourage individuals and groups to examine health behaviors and to make healthy positive changes
  3. Utilize motivational strategies to assist individuals to eliminate or reduce high risk behaviors and to adopt harm reduction strategies
  4. Provide information about health disparities that impact LGBTQ+ individuals with groups and individuals
  5. Facilitate presentations and distribute information on health and wellness topics in a variety of community-based settings

Key Performance Area: Documentation and Reporting

Performance Standards: The Health Promotion Specialist will:

  1. Complete written reports for each individual and group intervention provided
  2. Document the use of incentives by having individuals sign in receipt of any incentives received
  3. Keep records of interventions and activities
  4. Complete data collection reports in a timely manner as required by any contractor
  5. Maintain appropriate confidentiality of all records and information in accordance with the law and regulations
  6. Complete all forms required for tracking, documentation, and billing

Key Performance area: Teamwork

Performance Standards: the Health Promotion Specialist will:

  1. Work with the community services team as an effective team member
  2. Attend and participate in team meetings
  3. Work with others to develop creative solutions to identified needs and issues
  4. Support agency sponsored activities both internal and in the community
  5. Communicate verbally and in writing both timely and professionally
